and the Rockford Metro Areas. Our Clinical Associates have expertise
in business, health care and social services. The next few pages will
give you more information about our professional services and how we

Who
we are:
The Caring Connection, P.C. is a private practice in the field of
professional social work and counseling. Our associates are Masters'
degreed professionals who are dedicated to improving the quality of life
for hurting people. For us, that includes individuals, couples, the
elderly, disabled and their caregiving families.

What we do:
Our clinical services associates have a wide range of professional
interests and experience in the areas of Counseling and Coaching,
Eldercare and Disability, and Social Security Disability Representation.
We offer Assessment Services to assist the family in determining their
care needs; Care Planning to provide a "road map" to guide them
through the service delivery system; Implementation and advocacy to make
sure the process works for the benefit of the patient and caregiving
family; and follow up Care Monitoring to assure that quality services are
available when needed.
Following an assessment, many families find
that our Caregiver Coaching Services are needed, often just to explain
what's going on, in plain English. We help the family become better and
more savvy healthcare consumers. Our Eldercare and Disability Services
have an important family support component, too. 1 in 8 people over the
age of 65 need/receive home health care. If it weren't for family
caregivers, almost all of the current homebound population of elders and
disabled (all ages) would need institutional care at a cost of over $200
billion per year. We help families understand and cope.
Help! I have an Impossible
Parent!
Not everyone was raised by Ward and June or Ozzie and Harriet. Personality
traits and habit patterns (both good and bad) tend to amplify with the
aging process. This is often a challenge for the adult child who must now
be a care provider to a difficult and resistant parent. We coach the
caregiver and support the family of the Impossible Elder during this time
of life transition.
What about people who need
more?
When interpersonal relationships are not healthy, everyone in the family
suffers. Sometimes individuals and families have relationship problems
that require more time and effort. We provide psychotherapy services to
help heal hurting people and their families. For the homebound elder, we
do house calls. For the family member, individual, family and group
psychotherapy is offered at our office. We accept Medicare and all major
insurance plans.
